Why not Your first name and then your last, like if your name was Mike, "Hello, this is Mike Smith and I..."
Unless it's a friend who knows you.

Smith is more proper. If somebody else is introducing you. Then Mr. Smith is correct.

wel u dont use prefix wen addressin urself.But wen u want 2 sound formal u can say can i speak 2 sam this is smith on d other side.Wen sum1 else is addressin u he should say can i talk 2 mr. smith(if hes formal) else u can b urself wid frens n family rite

U should not use prefix when answering urself.

i would introduce myself as Smith.............
example when you phone someone than you
start with good afternoon my name is lets say
John Smith that sound much better than Mr Smith.
