I'm saying UK answers only as I'm from the UK myself and I should imagine other countries have different proceedings for registering with healthcare than we do.

I think you'll need ID, like your passport, and maybe proof of address, but you'll also need the name and number of your previous doctor. I've got a little NHS card that it tells me to bring with me when registering at a new doctor, so if you've got one of them, then take that too. They'll make you fill in a form I think, and I think the most important thing they want to know is your last doctor, so they can get your medical records sent over.
