I think I know where your heading with this.
Each country issues it's own passport. e.g. US Passport, Indian Passport, Egyptian Pasport etc
To travel to other countries you need a visa. A visa is issued by the embassy of the country you wish to visit. The reason for a visa is that it gives the country you wish to travel, the chance to check for a criminal record, links to terrorist organisiations and your potential risk overstaying your stay (illegal migration).
Then there are different types of visa's. For examply for America, a Tourist Visa is called a B1 Visa, a Business Visa is a B2 (these are usually combined), a student visa is a J1 and on it goes.

A passport is the travel document with passenger/ traveller ID. A visa is a form of "permission" to enter another country. A passport from the persons birth nation is issued to them and they get visas to travel to different countries.

Depends on the country you are going to. For example if I traveled from Australia to the USA I don't need a visa unless I want to work there.

The document they need to leave their country is called a passport. It is issued by the country of which they are citizens of.
The document they need to enter another country for a visit is a visa issued by the country they wish to visit.

A passport is just a passport for where ever you want to go.visit your post office or your library to get an application.don't forget to bring 2 photos id and your fees, in my state it's $75 if you are a US citizen, if not you will need to have your passport renew at your embassy here and you definitely will need a visa to go to where you want to visit.

It's always a Passport. Of course, in a foreign language, it may be called something else. In German, they call it a Reisepass.
