My son feels like his illness is physical and the doctors are missing something. They have run a few tests but I know they think it's in his head. He will work for a few weeks and then he's so tired he can't get out of bed. He doesn't sleep well and he says when he sleeps he feels more tired when he gets up and the longer he's up the better he feels -- at about the 8 hour mark he starts to get a little energy. We're considering mao clinic but we want to exhaust our possibilities locally before we invest more money.

Malnutrition due to a hormone disorder.
Grave's Disease (simple blood test for T3 and T4).
Chronic Fatigue Syndrome.
Depression.
Poor conditioning (doesn't exercise much, so has little stamina).

I suggest a full work up including CBC and hormone levels.

Sleep Apnea, Narcolepsy and Idiopathic Hyper-Somnolence are seriously worth considering. The last of the two would require medicines such as Modafinil which triggers those sleep areas of the brain to stay awake without the effects of "speed". Only a possibility worth looking into and possible Manic Depressive Disorder too.
Diabetes is another.

If he has bad habits, he may have a pulmonary problem. He must not be getting oxygen to the limbs which is a sign of heart problems.
Another possibility is diabetes.
Either way lends itself toward obesity. If he is hauling an extra 100 lbs it could be both.

Depression, perhaps. Depression can manifest itself with physical symptoms like those you mentioned. While it may be all in his head, it doesn't make it any less real or worrisome. Have you followed up with a psychiatrist? It can't hurt. If you are serious about finding a cause then all avenues should be investigated.
