There is no way to answer this question. The best pain med depends on so many factors. Different people react in diferent ways. So your tolerance and how well your body reacts to the med plays a big part. Some pain meds can make you feel worse. How your injured or ill makes a difference also.

The best pain reliever is the one that relieves your pain without any significant side effects.
Just like there is not one best car or best dish, there is not one "best" pain reliever.

Slightly silly answer: The best pain reliever is the one you haven't built up an immunity to yet. ;)

Long answer: If you experience chronic pain, you may need to rotate your pain relievers. I use Aleve until it stops working, then switch to Tylenol until it stops working, then switch to Aspirin/Ibuprofen until =it= stops working... lather, rinse, repeat. (Keep in mind that this cycle takes weeks/months).

Short answer: I've had remarkably good success with Aleve, personally.

It depends on the level of pain you are experiencing and whether you require prescription medicine or OTC analgesics will work.

OTC...aspirin as always worked for me.
