Gangrene would be an obvious answer when the blood flow is severely reduced to the extremities

There is also a syndrome known as Raynaud's Phenomenon (or syndrome). It may seem like a blood pressure problem to many, but it has some other symptoms.

Wikipedia defines it as this:

"Raynaud's phenomenon (RAY-noz), in medicine, is a vasospastic disorder causing discoloration of the fingers, toes, and occasionally other extremities. The cause of the phenomenon is unknown, but emotional stress and cold are classically triggers, and the discoloration follows a characteristic pattern in time: white, blue and red. It comprises both Raynaud's disease (primary Raynaud's), where the phenomenon is idiopathic, and Raynaud's syndrome (secondary Raynaud's), where it is secondary to something else."

warm them up in warm water to increase the vessle size and flow seek medical attention right now to avoid amputation later.

Don't smoke or do anything else that constricts the blood vessles to the extremities... keep the heart warm, drink lots of water, wear an extra T shirt to keep the core warm, when the core temperature drops the blood vessles constrict in the extremities further impeding flow, keep the core warm and the vessles will dilate or expand in the extremities to cool the blood for the core.

Raynard's disease, try wearing a good pair of thermal gloves when outside even if only for a short period,if it is causing problems go to the Dr's he will probably refer you to the hospital

I think it can be caused by Raynard's disease which is poor circulation in the hands.
You suffer from cold clammy hands. Even on warm days.
