Soak them for about 5 mins (or till all the ice melts) in normal water (warmer water if you want it faster) before you cook them.
I usually just put mine in this sieve thing which fits into a pot. Add some water, just enough to slightly cover the veggies and leave it for bout 10min. That should work fine.

I usually put them in a very little bit of boiling water. Wait until water boils again and let them cook for a few minutes.
It is important not to put too much water so that vegetables do not loose their characteristics.

I always cook my frozen vegetables in the microwave with just a little bit of water in the bottom of the bowl. they always turn out just fine.

Put them in a pot with enough water to cover the bottom & some seasoning & butter & cook on a low flame with cover on.
The longer you cook them the softer they'll be.
Stir them every now & then.
