The difference is in the laws. A person can be promiscuous without legal prosecution, barring children that this promiscuity causes...but that's another subject altogether. Currently, though, the laws state one can only marry one other.
Judgements of this law or lifestyle aside, that's the difference.
Personally I don't find polygamy disgusting or sick...I wouldn't want to be part of i...but to each their own.

Polygamy is by deffinition marrying more than one person. What Hugh Heffner does is just sleep with them. The difference between Hugh and a polygamist is that polygamists can create mental anguish by having children with multiple wives. Plus the American tax system is not set up for fielding families where there are multiple mothers.

Polygamy is not sick and disgusting. It has be a valid way to live in many societies and not only in having more that one wive but also with multiple husbands or a socially accepted lover.
This is also the case in the animal world, as contrary to popular thought most mammals are not monogamous and females of various species will often mate with a good provider but will reproduce with a different male.
The only reason that christianity places so much importance on 1 spouse is from Paul saying that a church leader should only have one wive...

One BIG difference is that modern polygamist take several young wives and have a multitude of children which are supported by the taxpayers because only one wife is legally recognized, while the others get all the public assistance money they can. Also 13 year old girls are too young to marry and have children of their own because they are still developing mentally and emotionally.
Heffner makes his own money through his very legal business and if he wants to invite 3 or 8 or however many young women to live with him HE pays his own bills, and those 20-something year olds are old enough to choose for themselves. Also since they are not having children if and when his girlfriends choose to leave they are able to do so.

Not everyone finds Hugh Heffner acceptable. I find him to be more toward the digusting end of the spectrum.
That said, the main difference is one is legal, one is not.
Polygamy in the U.S. often means underage girls are forced to marry older men and have babies, whether or not they want to. The man may also be a relative, so you can add incest to that scenario. Not a good thing.

The law is the difference. And it's a big one.
The only people who think that it is acceptable or macho are men who live that way. Women who live that way are drawn to the power and fame. If Hugh Hefner wasn't Hugh Hefner, I guarantee you he wouldn't have a "harem" like he does.

Hugh does not beat his women down mentally to make them feel inferior and that they can do no better, they are with Hugh by choice also and he does not loan them out to other men and he does not marry 13 year olds and start having babies with them right away.

lol, that's a good question. Perhaps there's not any difference.
It seems anyone who is morally against polygamy would also have to be against a non-monogamous, promiscuous lifestyle. That includes pre-marital sex, marital affairs, etc.
Sounds like that's a good chunk of the population there.

Because of the commitment involved in marriage that is absent in mere sexual relationships. You cannot make that commitment to more than one person. This holds just as true for unmarried people in committed relationships. Without that commitment, it's fair game.
