Depends on what country you are in but most countries have stingent laws where you have have to provide written and visual proof of identity for any information that is not in the "public domain".
Such information may as in here in Aus be the need to satisfy a certain number of points by producing personal documents form various sources along with photographic ID such as a licence. For more sensitive info- ie tax /medical etc you may need to also provide answers to a series of pre determined POI (proof of identity) questions which have previously had the answers documented (and sometimes checked for validity to see if the source is bogus) by the entity involved.

The laws on confideniality are quite stringient. For me to get my own medical records I need to sign HIPA paperwork. The only way around the HIPA paperwork is if a judge gives a court order. For my daughter's medical records to be released to the Prosecutors Office here in the State of New Jersey, I had to sign HIPA paperwork specifically designating each person who was allowed access to her medical records, which medical records and for how long this was in force. --- and that is the Prosecutors Office!!! They are quite strict.

therapist, ir that field, you lose your license, unless under the terms that the therapist or ie, feels that someone is in danger,
say you wnat to hurt someone, or if they are suicidal, by law that has to be reported. no loss of license.
doctor, has to maintain confidentiality unless instructed by patient or in writing. except in child abuse cases, or child molestations,
that has to be reported by law, no loss of license.
lawyer, has to maintain confidentiality, even tho i would make an exception, if someone says im gonna **** up the building. but i think even that is confidential as sad as it sounds.

as far as hospitals, they have to maintain confidentiality
unless in writing, by the court, period.

That it is confidential and if you want it to stay that way you should sign a confidentiality agreement

it can only be broken if someones life is in danger.
