Sometimes you don't know. If you miscarry early in your pregnancy, you may not even have been able to confirm that you were pregant. If you are past the first trimester, there will be some tissue passed, and some heavy, clotted blood. The bleeding will seem like a heavy period. There may not be much pain associated with the miscarriage -- again, that depends upon how far along you are. For the first several months, the foetus is very small, and so it passes easily, unlike a full term infant.
If you suspect you have had a miscarriage, see your doctor and ask for a D&C to ensure there is no tissue left behind, which could cause an infection and have serious implications for future pregnancies,.

If you are having a spontaneous miscarriage, you will probably have vaginal bleeding, abdominal pain, and cramping.
Bleeding may be only slight spotting, or it can be quite severe. Your health care provider will ask about how much you have bled—usually the number of pads you've soaked through. You will also be asked about blood clots or whether you saw any tissue.
Pain and cramping occur in the lower abdomen. They may occur on only one side, both sides, or in the middle. The pain can also go into your lower back, buttocks, and genitals.
You may no longer have signs of pregnancy such as nausea or breast swelling/tenderness if you have experienced a miscarriage.

A miscarriage usually begins with you thinking you have to go to the bathroom. Its almost like actually labor pains...you will bleed, and sometimes the sack comes out. It just depends on how long you've been in pain.

I had some heavy bleeding with lots of clots while I was pregnant. everyone said that I was miscarrying...so i thought I did.
then went the dr...and I'm still pregnant.
However heavy bleeding can be a sign of miscarriage...but not always.
So if you have any doubt..make sure you check with your dr

i had a miscarriage at 8 weeks and i experienced very heavy bleeding with lower stomach and back cramping.

usually cramping, bleeding and passing clots. but your water may break with no cramping or bleeding in which case you'll need a D&C. to remove the "products of conception" I delivered a 14 week fetus in the washroom after my water broke but didnt have a D&C for 4 days

you should have your self checked out to make sure that you have had a miscarriage if you are pregnant best advise is to get checked out

You can get really sharp pains and you can bleed. If you think you have, then call your doctor right away!
