An emissions inspection, or SMOG Check is simply what it says; an inspection of the emissions controls of your vehicle.
It consists of three parts:
1) Visual— Is everything that is supposed to be on the car present, and is it all complete, undamaged, and unmodified.
2) Functional— Does all of the required equipment function properly, and is the ignition timing correct. Is the "Check Engine" light functional, and it is off.
3) Emissions— A test to measure the 4 or 5 exhaust gasses expelled by the exhaust.
a) Oxygen present in the exhaust.
b) Carbon DIoxide in the exhaust
c) Hydrocarbons present in the exhaust.
d) Carbon MONoxide in the exhaust.
e) Oxides of Nitrogen (if the car is driven for measurements)
If all three parts pass, the vehicle passes and you are done until the next required inspection.

It is a check of the emissions control sytems of your car. Depends on where you live as to how strict they are. If you have to get one done, here are some tips. Put a fuel injector cleaner in your vehicles gas tank, and premium gas. DO NOT take it in to get the test done with the injector cleaner in the fuel though. IT WILL NOT PASS! You must run the tank almost empty, and then you can refill, again with premium gas, and go for a smog check. If your car is very new though, just take it in anytime, it should pass OK. The most common cause of cars with good emissions failing is that they have some sort of additive in the fuel AT THE TIME OF THE TEST.

checking for a mixture of pollutants, principally ground-level ozone, produced by chemical reactions in the air involving smog-forming chemicals. A major portion of smog-formers come from burning of petroleum-based fuels such as gasoline. Other smog-formers, volatile organic compounds, are found in products such as paints and solvents. Smog can harm health, damage the environment and cause poor visibility.

It is a check of the system in the vehicle to reduce emissions from your car. The performance of the catalytic converter is checked as well as how the EGR system is working as well. All of those sensors and O2 sensors, the computer, and so forth need to be functioning properly. Don't disconnect anything, it could be an automatic fail for your vehicle. In a nutshell, that is what it is all about.
