It depends on where you live. In many states, you can get them from your school. Some require you to go to the courthouse, a couple require you send a request to the state capitol. In a few states, potential employers have the forms that your school guidance counselor/academic counselor and your parents must fill out.

All you need to do is go to your local job placement center ask for a work permit have your parents sign and you're good to go
