The number is the measurement around the chest wall below the breasts. The letter is determined by measuring the breasts at the fullest point and for every inch that the breasts are fuller than the measurement of the chest wall, determines the cup size using the following rule of thumb:
A cup = 1 inch
B cup = 2 inches
C cup = 3 inches
And so forth ...

Someone who wears a 38b bra would be 38 inches around the rib cage and they would be roughly 39 to 40.5 inches around the bust.

People who don't wear bras (ahem, guys) get this wrong a lot. It's the letter and ONLY the letter (A, B, C, etc.) that describes the size of the breast. The number has nothing to do with it.

It means she has a medium/large rib cage and small/medium boobs.
