I think the basic rule of thumb is to work your way from the inside out. But I am just a redneck who doesn't eat fancy dinners with more than one fork.

The general rule if you don't know the difference is: start from the outside and work in. (In the States, at least...)
So, whatever is served first (in the States, salad), you use the outside stuff first.
Spoons in front of the plate.... well, if you're at that fancy of a place, please invest in some etiquette lessons before going!
Remember to bring the food to your face, not face to food, and don't chew with your mouth open... but I digress.

I actually know where everything is and how to use it. If you had spent years as a child getting yelled at by your grandmother because you weren't buttering your bread the right way, you would know too!

I know the knives and spoons, but I use any fork for forking.
If it hurts people so much to see my table manner, they can always turn around :)
