They don't. Needles are made sterilized and they don't reuse the same ones.

I don't know they are gonna die anyway they should save some money and use the same one over and over without sterilizeing it

For the same reason we have to make sure they are healthy enough to kill. Lawyers are the cause. We have to be sure that the execution is not cruel and unusual punishment.
