During the 1800s, when hanging was more commonplace as a means of execution, there were a lot of instances where the person was decapitated due to the poorly designed rope, such as length, the weight of the person hanging is a big factor, and the type of noose that is used. If the rope is too small, then decapitation can occur, with the body being too heavy, if it is too long, then the person can slowly strange, so methods were put in place in the early 1900s to be sure that it would occur as frequently as it once did.

Yes--when a person is hanged, there are sandbags or something of that nature, anyway something heavy, attached to the rope so that the person does not hang there strangling. It's really meant to make the execution more humane, if you will, but if the weight is too heavy a person can be decapitated by the rope

If a noose is designed correctly than your neck will snap instantly, the only time someone strangles is if it is done incorrectly. If a rope is made out of artificial fibers than it can cut the flesh of the neck causing the head to come off. It happens, it is rare but it happens.

If the rope is too long, the head can come off.
If the rope is too short, or the knot misplaced, the person just strangles.
If the rope is the correct length and the knot is correctly placed, the neck breaks and death is fairly instant.
