yeah thats pretty racist. its a reference not only to a black stereotype of drug abuse, but also slave work on cotton plantations. Did you really need to ask?
and tell your 'friend' its not a very good joke either.

Yes, it is a bad joke. And, if your friend would notice, just about all the medicine manufacturers that produced a pill product, have removed the cotton out of the bottles...it was there for no reason other than to keep them from moving during shipping. They actually saved a ton of $$$ by removing it.
