No. All you need is your account number. Endorse the check for deposit to your account only. If you want, enclose a note to deposit the check to the account number you have.

I'd say no as a rule but, i myself would include one just because for them to have your name and account number all in one place, less chance for error.

You should include it so they know what account you want the money placed in.
