She was thrown through a wall by the demon Shacks. They didn't elaborate on what happened to her, just that both she and Piper were hurt and by the time Leo healed Piper it was too late for Prue.

She was thrown through a wall by Shax, a demon assassin, while Leo and Phoebe were in the underworld and could not get back in time to save her and Piper. Had Phoebe been there they could have vanquished Shax. Had Leo been there he could have healed them. By the time they got back Prue had left her body and Piper was about to. Leo saved Piper and the sisters buried Prue when they could not bring her back. They said a lost sister spell but that found them Paige instead. Later they were told Prue moved on with her destiny and they would have to wait to contact her. Meanwhile they needed to train Paige so they could vanquish Shax. The whole thing was part of a deal the Source made with Phoebe to protect the sisters and have a truce. The Source lied and sent Shax after the girls anyway. As was Prue and Piper were going to die anyway. They were both going to be shot, Piper by a Sniper and Prue by a swat officer. At least this way one of them survived.

Well, Shax one of the Sources Assassins was after a doctor and Prue and Piper were trying to protect this doctor but Shax "Blew" them into (through) the wall and by the time Leo got there Prue was already dead so he healed Piper
