In my experience, it means that you're about to hear something you're not going to like.

I usually say that before I am going to say something that they may not like, and I am tempted to lie to them to avoid that. But, I don't, I come out and tell them the truth leading with, "To be honest with you....."

It means they might say something hurtful, but they're being honest.
