Just claim her yourself. There is no form that I know about. He needs her social security number to file. Does he have that? If he doesn't support her, why? If he does support her, he has a right to claim the expense just like you do.
If you both claim the baby, you will likely get a letter from the IRS. Don't panic. Just go in and explain that you are the primary care giver and entittled to claim the child. They will persue him.

I think I know what you said.! Whatever you file, 1040, 1040A, 1040 EZ, go to the Post Office and get the Booklet with your number on it. Read. The booklet tells you what to do , how to do it, and when you must file...Not hard, just read. It is self explanatory.....

If you have custody of your child you just plain claim her on your regular tax form. And please learn to type in english.
