To use a program called "Echolink" (A Ham Radio comm. program), my modem must access UDP ports 5198, 5199, and TCP port 5200. How do I set my Actiontec Model GT704WG modem to access these ports.

Log into the web interface of your modem....I guess it's http://192.168.0.1 since you posted that in your title. Look around, there should be options in there for Port Forwarding.

On each line put the starting and ending port for each port.
i.e. Start: 5198 End: 5198
Make the Protocol UDP
Put the private IP address of your computer

Your Actiontec GT704WG is not a modem. It's a router. Typically you access the router's configuration page by opening a web browser to it's IP address (192.168.0.1 I assume?) If it's not web-based, you'll have to telnet in (open a command prompt and type "telnet 192.168.0.1"

There are two steps. First, you need to make sure that a firewall is not blocking those ports. Check the firewall feature on the router, if it has one enabled. Make sure that these three ports are not being blocked by the firewall (they probably would be by default if the firewall is turned on. You will also have to disable the firewall on your computer, or at least ensure that these three ports are not being blocked there as well.
Second, you need to forward those three ports to the IP address of your computer (from a command prompt, type "ipconfig". This will give you the IP address (192.168.0.xxx). The port forwarding feature will be listed as "applications and gaming", NAT servers or "port forwarding". Check the documentation that came with your router for details for that specific type of hardware.
