To say it's the Quran is too simplistic. That's the same as saying the bible is behind the Holocast.
It's a complex blend of social and economic factors. Does religious extremism play a major role? Yes, but people generally don't turn to extremism, en masse.
Economic disparity - in the Middle East, there's a lot of poverty, and there's a lot of uneducated poor people frustrated by their lot in life. There are also a rich, ruling elite that doesn't share in wealth, and, for the most part, they are linked and enabled by ties to the West.
The fact that there is a legitimate issue with economic and political equity and the fact that the majority of people don't have a say in their lives leads to much frustration and bitterness.
Having a small kernel of truth in the idea that this is enabled by the West, that we have (or our corporations and ruling classes have) profited from these inequities while turning a blind eye makes it much easier to funnel those frustrations towards an "enemy" and to use the power of the mob. It's always easier to maniuplate a poor, frustrated, uneducated mob.
Hitler did this in the 30s. The Germans weren't on board because they hated Jews and wanted to perpetuate a Holocast. They were frustrated, there was a kernel of legitimate blame for their plight in the draconian terms that ended WWI, and all that made them ripe for scapegoating others for their problems and looking to a demagouge for solutions.
There is no "root cause," but rather there are root causes. Many factors that are necessary, but not sufficient.
That's why simple sound-bite solution won't solve the problem, because many of the underlying factors will still be there, or will be made worse.

I think it is mostly politics. Maybe the root cause of the hatred is the fact that the US supports Israel.
A historian would be best placed to answer that one.
That said, according to Michael Moore, Osama and Bush were friends at one time but they fell away. Same with Sadaam. So, what happened? I think politics happened under the table. That's what needs to be figured out. Actually, it probably has but is hidden in some book that I haven't read.

The hatred has it's basic roots in the teachings of the Quran as others have mentioned. In the early 70s, the radical perversion of the Quran really took off and the west has been viewed as The Great Satan since then. I believe that term was coined in the very late 60s, early 70s and the west truly is the only thing standing between them and their goal of religious dominance.
And to the delusional fools who believe their hate was born in the years of the Bush family, I suggest a little reading and research. The first known attack on America happened during former President Carter's administration. The terrorist activities was ignored until 09/11/01 happened. Look what it got us.
As we speak, they are teaching their doctrine of hatred in the heartland of America.

The root cause is that they are a bunch of mindless idiots hell bent on living like cave men. Anyone with any sense over there finds a way to move to a civilized part of the world.
The 'cause' they pin it all on is Israel, and Western Nations' support of its existance. If we renounced Israel tomorrow all the Arab nations would fall at our feet and give us their oil for free. Then in about six months they'd find some other reason to hate us again.

It started with the Quran. Accordingly, nearly everything that Americans enjoy is sinful - comparably to the bible as well. They train their young to hate everything western civilization represents. The only difference now, we have given them a valid reason.
