Walgreen's sells both prepaid MasterCard and prepaid Visa cards. That's the best way to go with teens. Put the money in for them and when they spend it all that's it. they can't charge any more. Stops them from being able to run up a large credit card bill. Don't trust the parental consent because a lot of times the credit card companies will OK the charge without parental consent.

I'd go with the first suggestion, depending on what banks are in your area. You will most likely need to have your name on the account and if you have other accounts with them, like your own checking account, that helps. However, opening a deposit/debit account does not affect your credit whatsoever, unless you have collections recorded from severe overdraft, so your child will not build credit using this method.
Also, don't be surprised if you have difficulty with some places as anyone under 18 having a VISA or MasterCard, even a debit card from a bank, can be controversial because a lot of online companies use credit cards as age verification that the person is 18+. Also, consider a rechargable MasterCard or VISA gift card that you can refill for them. I know VISA makes a special kind of pre-paid card for teens.

Almost any bank will have a parent consent credit card. Just ask your bank and if you have good credit then they should let u get one for you teen.

you can't exactly get a credit card, but you can get a debit card that basically acts as one when you apply for a checking account in Bank Of America. the teen must at least be 16 years of age though and you need an ID or passport. The card can be used to pay at stores, and ATM machines...its basically the same thing.

there are two ways...
if you are wanting a certain amount on it, so that they can't spend a whole bunch, than go to the grocery store, and look at the gift cards...there is a Visa gift card there, with different amounts on it, and it i good anywhere that accepts a credit card...the only difference is that there is no bank involved in the visa gift card...and no tax on it ether...so lets say you go and buy a card for $50, and you go to pay...it's only $50, no tax on that...
otherwise, you can go to your bank that your teen has their account at, and tell them that you would like a credit card for that account and ask them to only allow a certain amount to be taken from the account..talk to the bank attendants for more information on that...my dad did both of those for me, and they are both working fine, and he controls how much i spend and where i can spend it...

Best way I know without any questions asked is to do the application online. You can provide all the information and possibly be approved straight away. There is a competitive market out there at the moment.
