It means a specific person who had credit with a company (store, whatever) has had there credit approval removed. It appears that the person shared credit there with someone else, that the someone else requested that person's credit be suspended, and that the request was approved by the person who had extended the credit.

Sounds like they closed your credit. The grantor would be your credit card company who you owe money to. The are also the subscriber because they subscribe to the service that reports your information to the credit bureau.
Sounds like you owed them money and could have not paid it on time, and they cut off your credit line.

It means that a loan facility was closed by the bank because they weren't satisfied with the way the cflient was maintaining it. This was reported to the credit reporting company and is on the person's record at the bank's request.

It was put there to warn future potential lenders that the person is considered a bad risk.

This means that the person who gave you the credit close the account and they are also the person who reported this information to the credit bureau.
