It was a book written a long time ago when people had to make up a reason as to why they were on the earth. People who don't believe the stories in marvel comics are true don't insult spider-man; they know he's just a story.
However, they're likely to insult other people - real people - who are gullible enough to believe he really exists.
And if believers threaten violence, or try to restrict the freedom of nonbelievers - things can really get out of hand.
