I'm trying to re-new my permanent resident card, but on the form it is asking for my "port of entry" where can i find this infomation?

That is just where you cleared customs at. As in the city, airport, seaport or border crossing.

When you got your big immigrant packet in the mail and you took it to an airport, seaport, or land border crossing in the USA, to be processed; that port is your port of entry.
