you need to get his teeth cleaned. I take my dog twice yearly just like myself to get her teeth cleaned. they do sell products at the pet store to put in the dogs water to help with the breath.

You are feeding him a totally inadequate diet, lacking in most of the nutrients he needs. Milk and rice are NOT acceptable dog foods. Get him onto a good quality kibble right away, with no more than 24% protein and 14% fat. I cannot emphasize strongly enough that you are endangering his life and health.

Brown rice? Milk?
Bad breath starts in the stomach, so make sure what you're feeding him isn't upsetting his digestive process.
Get his teeth cleaned, definitely, and look for dog-breath biscuits (they exist) to give him as treats.
You as the pet owner should also brush his teeth to keep them healthy. Don't use human toothpaste (hey, I've seen people do it) because it'll make him sick.
They even make dog breath strips now, which worked on my Beagle (who has chronic halitosis.) See link.

For one thing he needs a better diet..if you want to feed him raw or homecooked taht's great, but he needs it to be proeprly balnced and conatin the right vitamins, minerals & protein. The diet you are currently feeding is inadequet in many areas.
Next he needs a complete vet exam including his teeth & gums.
If he is an older dog tyroid, kidney & diabetes tests may be in order as well.

At like Petsmart or Nj Pets (or any other pet store) they have little doggy breath strips (basically like Listerine Strip, sorry if i spelled it wrong) and it really does work well. They make not like it that much, but it leaves their mouth smelling minty fresh =]
