I know one "must" file the income from New York State Disability as you are paying taxes on it, but my question is if this is included with the W2 as the disability was paid through my employer or under third party sick pay?

disability pay ,or workman's compensation ,as it is also called, when payed by comp,is not subject to state or federal tax because it is payoff to an insurance policy.disability pay is the same ,only with a private insurer, also is exempt to state ,federal &local taxes. it is unemployment compensation that is taxed as income.

Enter it on the wages line. And yes, FICA is for social security. Disability payments before normal retirement age are treated as earned income.

State disability income is NOT the same as workman's comp, as another responder said. Workman's comp is for on-the-job injuries. Not all states have a state disability plan.

I don't think disability is taxable to IRS, call IRS 800 829-1040 and/or go to irs.gov. Yes FICA is SOC SEC.
