Civil engineering is the general term for any engineer who designs public structures (including buildings, dams, highways, etc.) Structural engineers are a subgroup of Civil and are those who specifically deal only with the actual design of the structure's "bones" -- what gives a structure its fundamental shape and strength (mainly steel and concrete).
If you thought of a building as a human body, the Structural Engineer would design the skeleton, the Civil would design the muscles and skin, the Mechanical would design the lungs, digestive and circulatory systems and the Electrical would design the brains, sensory organs and nerves.

First you get a civil engineers lincense. Then if you want to design buildings over four stories tall or if you want to design schools you need to take another test to become a structural engineer.
