at one point it breaks up and you see little squares

doesn't happen with other DVD's only with certain ones

Could the DVD have a defect, or could my DVD player be bad

What you see are "macro blocks" ,16x16 pixel squares used in encoding DVD movies. In MPEG (used in DVD movies) each frame is encoded using info from past farmes. If that info is corrupted (scratched or dirty DVD) then the player can't reconstruct the frame, so you see these blocks.
Since it happens only on certain DVDs, make sure they are clean and without scratches.

Try cleaning the DVD....it might be as simple as a fingerprint that causes the pixelation.
Simple use a damp cloth and wipe from the center of the DVD towards the edges until it is all clean.
This happens a lot to me with rented DVDs
