I have been suffering with pain both top and bottom - can't decide where it is coming from - on the left side of my mouth. Have been to the dentists four times now - this has been going on for 18 months intermittently. Last time she xrayed all top teeth on the left and said there is no decay - they are all vital. She thought it was the back tooth which has very little enamel on it. She put some gel on it and advised Sensodyne Pro Enamel toothpaste. It quietened right down and has just been a bit sensitive for a couple of months. But now it's back again - four sleepless nights. It is worse when laying down and seems to pulsate a bit. Then in the daytime it goes from a dull ache to nothing and back again. Any ideas what this may be? I am so totally desperate for help - my old dentist who I trusted implicitly retired last year and this new one is very nice - from Poland - but I don't know whether she is on the right track or not. HELP!!

To me, you are describing an abscessed tooth. This is almost diagnostic. If you have inflammation in an area, there is more blood flow to that area. If you place that same area lower than the rest of your body, there will be even more blood in the area. This makes the area start to throb. Think about it. If you sprain your ankle, you are supposed to sit or lie down and elevate your foot to keep the excess blood from accumulating there because it would cause throbbing. If you are passing out because your nervous system has prepared you for "fight or flight" by dilating all of your peripheral vessels, which deprives your brain of blood, you are supposed to lie down with your head lower than your body to restore blood flow to the brain.

If someone has an abscess and I have just started a root canal or if they are going on antibiotics for a few days before I remove the tooth, I tell them to sleep on an extra pillow to elevate their head. If that doesn't work, they can move to a recliner or the sofa so they can keep their head really elevated. This will minimize the blood flow to the area and therefore minimize the throbbing and pain.

So - elevate your head when you sleep and get back to the dentist. Sometimes it is hard to find which tooth is bothering you in the earlier stages, but this sounds like it should be obvious by now. If she can't find it, get to an endodontist who will surely be able to locate the source of your trouble. I hate to say it, but I think you might be right about your new dentist missing something.

It may be an abcessed tooth, but it can also be a symptom of clenching and grinding. Often you can generate so much pressure clenching and grinding that you can mimic root canal pain. When you gently bring your teeth together so that there is just the slightest touch - do you touch first on the left side? When you bite down does it hurt on the left side? If so, you may need to have your bite adjusted - the teeth slightly filed down to relieve some of the pressure when you put your teeth together. I would definately try to rule that out before starting root canal treatment - unless the x-ray shows definitive infection at the roots (which will look like a black area at the tip of the roots in the bone).

The position of rest is lips together, teeth slightly apart. Do you wake up with your teeth clenched? As you go through your day, when you are concentrating on something else, see where your teeth are. If you are touching, you are clenching and if you are clenching during the day - 100% you are clenching at night. If you find that you are doing this your dentist should make you a night guard. This is an appliance that will protect your teeth from the force of clenching and grinding.

Try to keep your teeth separated today and for tonight, until you see your dentist tomorrow, go get one of those cheap moldable biteguards (they are ~$3). Cut the back part of the biteguard off and mold it so that it fits just on your front teeth, and prevents you from touching your back teeth together. Also call your dentist's emergency line and ask them to prescribe you some pain medication to get you through until tomorrow. Good-luck and hopefully you will have quick resolution of your pain.

What about going out to the the Pharmacy or CVS and OTC you can find Ambusol, it should be in the tooth cleaning section, it is a gel and if you rub it on the tooth and gums will take the pain out. Then you can see the Dentist on Monday. It is amazing how toothaches develop on weekends or Holidays. You are free to find a nother, perhaps more experienced Dentist, at this time if you choose to do so. Best of luck, toothaches are no fun at all, there is help however , and get well soon.

I'm going through the same exact thing. it's been about a year and 1/2. I went to the dentist. they xrayed found nothing. I would hurt so bad at night, I would put a aspirin on it and that would help for about 15 minutes.. I did not know which one it was. but it was on the left side in the back. during the day it was not bad. I went to my family Doctor. he said my pressure was up. gave me antibiotic and meloxican and the pain stopped . now it's back again. as soon as I close my eyes the pain is back.. I was up all night. I sat in a recliner. but when I fall asleep the pain wakes me right up. I rub it and put some ice chips on the side and that help some.. I am tired of this and want to know what is this.. my back teeth do not have any enamal and it chipped.. I know your pain.. .

if you feel pain from your gums, it is possible you have abscess, which is puss. If that is the case all you would need to do is have your dentist drain it, and you would feel better immediately. I don't see how your dentist could have missed that though.
