It is just a phrase, do not take it so seriously. It means a sincere gift, without hidden intentions.

To give from the heart is to give to another something that you know down deep it's what they need or want without pertaining to value.
I once had a girlfriend give me an enlargement of a photo she took out of my wallet of my son whom I loved very much. It didn't cost much but it was one of the best gifts I ever got in my life. She gave it to me from her heart.
To give someone your heart, you are giving them your total love and affection but no one else.

It simply means your giving yourself as a whole to that person. Meaning you Love them enough to give them your heart! Sounds kinda strange but its true.

Give from the heart means that you don't give based on intelligence (Like, for example, this person is really rich, so I must give them something expensive). It means to give them something sentimental, that you know they'd like.
