It means that two people are seeing each other, or dating. The level or the relationship is the thing that people generally don't agree with.
A relationship is always something between two people. It dosn't have to mean dating, for example, how would you describe your relationship with a parent, sibling or friend? It's a word to describe an acquaintance.
Usually though, when people say it they're talking boyfriend/girlfriend, though, as I said, many would disagree on the amount of commitment implied by the word.
