Scott S.... find me a place where I can make over $3M a year and I'll be there! Ha! Not likely.
Really it depends on so many factors that it is impossible to say.
Is the hypothetical practitioner working in private practice or an academic practice? What region of the US? What is the payor mix of the patients? Does he/she accept Medicaid and Medicare, and is he/she participating or non-par with the local insurers? What is his/her malpractice insurance premium? Do you want the gross collection of fees or the net (take-home) earnings after he/she has paid for insurances, office staff, billing fees, equipment, etc.? What about the $100-300k he/she owes for medical school debt? Can we subtract those payments out?
Every doctor is different, and most won't divulge their income.
I won't.

It depends on the specialty. Surgeons do better than diagnosticians. I also does depend on location and the age of their patients, and the HMOs those patients belong to. People over 65 are on Medicare and doctors are limited as to what they can charge the patient.

Isn't this going to vary from country to country? City to City? Where are you? I'm guessing North American b/c you used "specialized" not specialised.
