I would take up the toilet, you might need a, riser/spacer that you put inside the ring to lift it up a 1/4 inch, put a new wax seal in also. the vanity doesn't matter if you plan to never put in a new one, or then the measurements will be different, as setting back down.

All I can say is: its probably not a great idea to put it right on top. It becomes more likely for it to come up. And then the process of taking up a layer of laminate and a layer of ceramic tile would be that much harder when it came about. I think the only way to make that work without taking up the ceramic tile is to make an air tight seal between the 2. I couldn't give you any ideas on how to do that though.

you have to remove the toilet but the sink if it is a cabinet just needs a kick board to cover small cracks. i should look like the tiles are under the sink, If it is an old fashion sink then you need to pick it up like the toilet

My husband does tile, he always pulls up the toilet, he says it looks better, But I'd also imagine if you had toilet trouble in the future and had to pull it up, you'd mess up your floor. The sink, you can just go around.

Yes you have to remove them if you want it to look nice. Unless it's a sink built into a cupboard you can lay the tile around it.
