I'm sorry to hear that things are going so badly for her. I've read that certain anti-depressants such as Paxil can trigger manic episodes so you really should talk to the doctor who prescribed this for her about this right away and if you don't get a good answer then please go to another doctor or therapist!

According to a Wikipedia page on anti-depressants, "In certain cases, an antidepressant can induce a switch from depression to mania or hypomania, can accelerate and shorten a manic cycle (i.e. promote a rapid-cycling pattern), or can induce the development of psychosis (or just the re-activation of latent psychosis) in a patient with depression who wasn't psychotic before the antidepressant."

Almost always a person with bipolar disorder is prescribed a mood stabilizer such as Lithium or Depakote and anti-depressants can be prescribed if the depression is especially bad but this has to be monitored, especially if the person's symptoms are worsening.

Maybe try couples therapy to work on communication, etc. It might help you better understand whats going on with her and vice versa. If inpatient is necessary, find a good program like Sierra Tucson or something similar.

Obviously, you need to find another counselor. The meds aren't working for her and neither is the counseling. You need someone who can help her, tweek her meds and give her the right counseling for her needs.
