When my aunt was about 35 or so, her eyes started going and she got some reading glasses. When they would visit, she would insist that after a cocktail, she couldn't see very well so my mom, as a joke, went to the kitchen and got one of those round seals from a milk bottle and brought it out to her as a monocle. My aunt put it on and insisted that she in fact could see better.
After that, whenever we saw her, she would ask for her monocle.
So, when it was time for her to turn 40, we collected 40 of these things and wrapped Popsicle sticks to them, wrapped them up and gave them to her. Her family, and mine were laughing outloud for minutes because there had been 5 years or so of buildup by this point.
I realize that the joke would be lost because there is no history, but perhaps look for something like that - is there some kind of way to turn an inside joke from years past into a gift?
However, I've re-read the other posts and many have a point - women can be sensitive about their age. For that matter so can men. I know between my sister and myself, we're pretty brutal. I nailed her bad when she turned 30, but then when it was my turn, ouch! That hurts! Tit for tat in that case, but we are siblings - neither of us care. My sister and I basically operate on the idea that "Hey, leave my (sibling) alone! (he/she) is MY emotional punching bag! Back off or you are dealing with me."

Does this person like "gag" gifts? If so, then look around. Maybe you will find something.
Some people have a difficult time when they turn the "big" ages- like 30, 40 and 50. Cards are o.k. Some of them can be "mean."
If you are industrious, you can do some research: what music was popular 40 years ago? How much did a loaf of bread and a gallon of milk cost?
There are booklets you can buy that cover those. These booklets aren't "gag" gifts, but they go back to the birthdate of the individual and are a bit nostalgic. Maybe some "gifts" or "candies" that were popular in the 60s would be appropriate?

I don't now how he pulled this off, but a couple of streets down from me a few years back... this guy hired a 40 piece marching band to play " Happy Birthday" on his front lawn for his wife at 6:00 AM. The neighbors weren't too thrilled, but it sure was clever.
Make a tombstone out of cardboard that says RIP and put it on her lawn.

I can't think of a good gag gift, but when my sister-in-law turned 40, I sent her a sympathy card!

think twice-unless she has a very good sense of humor,these jokes are usually sooooooo unappreciated by women! it could lead to bad feelings between u.i say,gr8 ,big "dont"-especially if its ur wife!she will never 4give,and u will never hear the end of it!she may smile at the party when people are around,but when she gets u home,she will either yell or cry-probably BOTH!ur taking ur peace of mind and putting it on the line!
