Bad breath can be the sign of dental caries, or the processes which are destroying the pulp of the teeh(the space inside the teeht). It also can be cased if there is a fistula with purulent liquor going out. Some gastroesophageal diseases can have it from numerous of symptoms. I sugesst to visit the dentist first, and if it is all OK with Your teeth, then go to otolaryngologist to see the sinuses and ears and then visit the specialist for gastroesophageal diseases. But I think the problem is in teeth.

Bad breath can be the sign of a variety of illnesses if typical remedies like mouth wash don't work. Consider dental problems, sinus infections and gastroesophageal reflux. Try consulting with your regular doctor and/or dentist. If they can't find the problem then an ENT (ear nose and throat or otolaryngologist) might be able to help.
