Check with your local court house on the paperwork needed to do this, the court will give you 2 certified copies of the paperwork when your name is changed, you keep one and you will send the other certified copy of the name changing paperwork along with your original birth certificate to your state of birth, to the department of vital statistics requesting a name change. You will be given a NEW birth certificate with the new last name..As for cost you will need to ask the local court for their price and also contact the dept. of vital statistics of you birth state on their price. You might even be able to check this out on line

I was 18 years old when I changed my last name to my stepfathers last name. In 1993 it cost about $75.00 and no, you do not need a lawyer, .I'm pretty sure I did this in Probate court. Stop by your local county office. You will fill out a form petitioning the court to change your legal last name. You will have an option, to change the last name on your birth certificate. I choose to keep the name I was born with and only change the name on my drivers license and social security card. Your court date will be sent by mail, and when you appear in court the judge will ask you questions regarding your reason for the change.
Also if you cannot afford the fee you can petition the judge to wave them.
Hope this helps!

I'm not sure about whether you can do it on line or not but when my brother and I had our last names changed to our step-fathers last name we had to go to court and it cost something like $150 a piece for us to get our names changed. Other than that I really don't know what to tell you.

I found this information:
legalzoom.com- they'll do it for a kid for $165.00 and I think $135 for an adult. , and you'll get documentation in 2-3 days. They'll do the whole thing online, and apparently you save $773.00 off of working with a lawyer.

inamechange.com-name change kits that are geared towards specific states. You have to find your state (warning-list is small) and use their specific information

1stoplegal.com-has a full list of states plus D.C. and contains an accurate kit for each state, but I think you have to go to wherever you would change the name yourself, as opposed to an online option.

namechangelaw.com-diy forms from every state. For $39.95 you can do it yourself, and for $99.95 they'll do it for you.

I found all of these-and more-in a Yahoo search-changing names. Do that if you need more info. Just remember to use adult forms, not minor forms.

Good luck!!

i got my name changed when i was in the military to my step fathers last name.
all you have to do is go to the court house (call first) tell them you want to change your name.
they will tell you where to go(cause it might not actually be in the court house)
you will fill out paper work that says you change your name form/too and it will cost around $100.00 (at least it did in Cincinnati)
you don't have to go to court, but it will take a few weeks, cause it has to be declared in the news paper, somewhere , of the change.
after that you get the official paper saying your name is now changed.

Talk to a lawyer that handles divorces, maybe they can help you, or guide you the right way.
