Well,
you need a blender!
Vanilla ice cream
unsweetened coco powder
some milk
and few ice cubes!
put them all in the blender and you will have a good tasting milkshake!
Or for lighter version
Blender
Milk
Coco powder
sugar
ice cubes
*all ingredients depand on amount you need! and sweetiness that you desire can be countroled by you, as well is thickness!

Here's your answer: From a New Englander at heart. There's a difference between a Milk Shake and a Frappe. So if you're ever in the area, you've got to know the difference when ordering one at a restaurant, even McDonald's! Here goes...
A Milk Shake, a chocolate one is made with mixing milk with chocolate, either chocolate syrup or chocolate powder.
A Chocolate Frappe on the other hand, is made the same way as a milk shake, but you add a few scoops of ice cream, preferably chocolate if you're going to have a chocolate Frappe.
Frappe pronounced "Fr - ap" with a short "a" sound.
Depending upon how chocolate you like it and how thick you like your drink, depends upon how much chocolate and ice cream you add. You can't go wrong, only if you don't add enough of either the chocolate or the ice cream. So be generous!

Put some ice cubes into a blender, add 2 big scoops of chocolate ice cream and some milk. Just blend it until it's nice and smooth but thick. If you don't have chocolate ice cream, then use Vanilla and add chocolate syrup.
If you want to try something really yummy, add 1 whole chopped banana and you will have a delicious chocolate banana milkshake.

A different but even yummier approach is:
2 scoop vanilla Ice cream
1oz whipping cream whipped
1-2oz Hot Chocolate
mix all together with a whisk in a bowl
add some kirsch or mint schnapps

You can just blend chocolate ice cream with a little milk, or you can make chocolate milk with cocoa mix, freeze it partially, and then blend it. I am sure you can find lots of variations online.

Chocolate syrup or powder, milk or cream and ice cream. Blend to the thickness you like. Add more or less ice cream depends on how thick or thin you like it.

Comeon now, Its only a glass full of chocolate icecream then fill it about half full of milk and mix it well for a thick one and more milk for less thickness.
