I'd like to know if every state in the United States requires its drivers to be insured by law.

they are some states that don't require you to maintain car insurance on your car. if you have a loan/lease on the car generally the people who are financing the car to you will require you to maintain full coverage though

45 or 46 out of 50 states require both insurance and liability.

No, in New Hampshire you are not required to have auto insurance. Also in South Carolina, Tennessee, Wisconsin and New Hampshire you are not required to have liability either.
