"Most vehicles in the Portland metropolitan or Medford areas must pass a Department of Environmental Quality (DEQ) emissions test as part of the registration tag renewal process. Most vehicles new to Oregon must also pass the test prior to being registered in Oregon."

So, if the car is new to you, then yes, it will need an emmissions test if you live in either the Portland or Medford areas. The second link will let you enter your zipcode to see if you are subject to the test (but it looks like you are). If you already owned the car, then the test should have been done in the last two years already and you probably don't need to have it re-tested until it is time to renew the vehicle license.

Yes, all vehicles must pass inspection. Typically these days they read the diagnostics from your car's 'computer' rather than actually testing the exhaust emissions though.
