Steven Greenstein:
"סטיבן גרינשטיין"

Jan 8th 2000:
"השמונה בינואר 2000", or "ה-8 בינואר 2000"

~
English to Hebrew transliteration has no obligating "rules", but there are some accepted transliterations for English letters:
"T" is mostly transliterated as ט (Tet). ת (Thav) is exclusive for "TH".
The diphthong "ay" is transliterated as יי (Yud Yud).
"SH" sound is transliterated as ש (Shin). ס (Samech) is exclusive for "S".

As for dates: the number of the day comes first in a plural-male form, then the month and lastly the year-
the eights in January 2000.

סטיבן גרינסתין
8 ינואר 2000

there might be different spellings as to the name since in the Hebrew aleph bet there are two letters that make the same sound. but that's basically how it goes.
