Yes - private health insurance is anything that's not paid for by the government. So if you bought a private policy, or if your employer bought it for you, it's private. Medicare, Medicaid, and some of the state CHIPS programs are public health insurance.

I can see why you might be confused. There are group health plans, such as you have through work, and personal health plans that you buy on your own. While “private health insurance” is sometimes used to describe policies purchased by individuals, it is more accurately used when describing non-publicly subsidized health plans. So, to sum up: you get group health insurance through work, which is private health insurance because it is not supplied by the government. Likewise, a personal health policy that you buy for yourself is private health insurance. Medicaid and Medicare are examples of public health insurance.

You are correct. Your employer provides you with a Group plan. An individual plan would be one that YOU would take out on yourself and pay the premium.
