Practice practice practice, either alone or with your partner. When alone, find out what really turns you on, the right spots to hit etc... When with your partner, never be afraid to tell him what you want. For one, he won't be embarassed or think that he's not doing it right, if anything he will think wow this woman knows what she wants and is not afraid to tell me.
I very rarely achieve the big O by penetration alone, I have to have clitoral stimulation, and these 2 combined will give you one hell of a orgasam.
The best postition to achieve this is with the lady on top and then you also have the oppurtunity to stimulate yourself.Something which will turn your partner on even more.

All 3 ways can be achieved by stimulating each thing. Practise with yourself and then practise with a mate. The more you learn about what pleases you the more you can communicate that to your partner. And that BIG O is awesome!

I leave out toys because from what I've heard alot of women over stimulate the clit and it makes it hard to do anything without one.
It's quite possible to get one from just using your fingers but you just need to work and let it build up
Some women expect to big O on their first few tries..
