IIf you're going for pure heat, any hot sauce that is made with pure capsaicin extract will be the hottest. Hot sauces are rated in Scoville Units.
The Scoville Heat Unit (sometimes referred to as the Scoville scale) measures the heat-producing capsaicins in a pepper. It was named for Wilbur Scoville, an early 20th century pharamcist, who developed a subjective taste test to measure the relative heat of peppers. Today, Scoville units are measured using high-pressure liquid chromatography (HPLC), which is more accurate and objective than the original test.
For comparison, pure capsaicin measures 16,000,000, jalapenos rate 2,000-4,000 and bell peppers rate 0.
With that in mind, the hottest sauce that comes to mind would be Blair's 16 Million Reserve. Search the web for this if you're interested. One drop of this in a gallon of chili or soup would light you on fire. :)
Good Luck.

Dave's Insanity Sauce is absolutely the hottest name-branded pepper sauce, since we're talking about sauces and not capsaicin, but there are many concentrated peppers sauces available from smaller stores. There's too much of a liability to sell super hot sauces on the national level, because people die from them. If you can find someone who makes concentrated habanero sauce, then you will hold in your hand the hottest sauce in the world. Just the residue even left over after bleaching containers can cause a horse to foam at the mouth. Producers must wear protective HazMat suits and gas-masks just to work with it. It can cause the throat of a person who's insides are relatively sensitive to such things to swell up and result in strangulation and hypoxia. It can eat the inside lining of your stomach and cause your intestines to bleed. I've even heard of people dying of heart attacks from the absolute surprise of the heat it brings on. USE WITH CAUTION.

There's a shop in Long Beach Ca called Hot Licks, they have 200 or so hot sauces. I asked for a taste of their hottest, they gave me a drop of Dave's Insanity on a toothpick.
That was too much- my mouth was burning as I ran for the beers!

Habanero pepper sauce is the hottest around, available at most stores. One drop in a kettle of chile will set you afire.
Each bottle has a warning label to read and follow. You better read it. Do not put a finger on the open bottle's neck and then stick it to your tongue. You won't be able to drink enough water.
While it's devilishly hot, the taste is not for everyone's palette.

Pure capsaicin (16 million Scoville units) is the hottest "sauce" you can buy. Dip a toothpick in it, then dip that toothpick in a pot of chili and you have enough to make the chili spicy. That's how hot it is. Don't mess around with it.

Blair's 6am pepper extract isn't technically a sauce, but at 16 million Scoville units, it has to rank among the highest heat contents on the planet.

there is a sauce called
Satans blood
It is over 800,000 scoville units of heat...which is alot seeing regular tabasco sauce is only 9,000 units.
This stuff will burn your skin and it has a warning label on it.
but...i just found this stuff...and at 999.99 a bottle this stuff would probably kill a small town of people.
4 MILLION scoville units of heat!
