When I was in hospital the medical staff would ask
Did you open your bowels today?(poo)
Did you relieve yourself today?(pee)
The staff would mark the chart accordingly.Nothing intrusive or embarrassing

defecate has a really negative connotation. Nowadays, they're saying "to have a bowel movement" and urinate still stands for peeing.

In Taiwan, however, they still ask you to poopoo in the cup. (heehee)

lol my doc calls peeing urinate too, but for pooing she says passing stools lol, don't even know if that's how its spelled, but yeah, that's how its said anyway
