Say some clever chap worked out how to bypass all security and was a master of disguise and could fool anyone and fired a nuclear missle from the USA to Russia (so Russia of course would think it was the USA), then went to Russia and fired a nuke from there to China. Then went to China and fired a nuke from there to the USA?

It's very far fetched but imagine it happened.
(The guy is a pilot and has his own concorde so can go around the world at his leisure no busy airports to worry about.)

Or to reword the question just say what happens if the USA, Russia and China got into a massive fight with each other?

I think that Russia has limited nuclear capabilities since the cold war destroyed the USSR (thanks Reagan!) so they could probably only blow up 1/3 of the U.S.
China on the other hand probably could not even reach the U.S. with its weapons because it has limited delivery capability.
The U.S. would probably be able to destroy both China and Russia quite easily and have some weapons left over. Some people project our nuclear capability enough to destroy the world five times over.
So it would be mass destruction on a level this world has never seen. Let's all pray this never happens! Fun to think about though.

wouldnt happen after the first one, all the silos would go into lock down no one enters or leaves.
