The answer to you question is yes. Stroke or "brain attack" ("heart attack" or myocardial infarction for the heart) or even intracranial hemorrhage may manifest with severe headache and the other symptoms of migrane attacks. However, I do agree with some of the answers that you got regarding having a neurologic deficit like weakness, paralysis, etc but not all the time. This will depend on the area of the brain affected. Also some of the signs and test mentioned in one of you answers which may also be seen but better let a doctor or a neurologist handle the testing because you may over diagnose or under diagnose or even misinterpret the examination.

However, in the case of your friend, I doubt that she has or had a stroke. First, I think that your friend is too young to have a stroke although there are some cases called stroke in the young. Also, one question is if your friend has any underlying medical problem or risk factors that will cause her to have a stroke. Migraines on the other hand are very common especially for females.

I would not think so, but here is a way to rcognize strokes

RECOGNIZING A STROKE
Thank God for the sense to remember the "3" steps, STR . Read and Learn!
Sometimes symptoms of a stroke are difficult to identify. Unfortunately, the lack of awareness spells disaster. The stroke victim may suffer severe brain damage when people nearby fail to recognize the symptoms of a stroke.
Now doctors say a bystander can recognize a stroke by asking three simple questions:

S * Ask the individual to SMILE.
T * Ask the person to TALK to SPEAK A SIMPLE SENTENCE (Coherently) (I.e. . It is sunny out today)
R * Ask him or her to RAISE BOTH ARMS.

NOTE: Another 'sign' of a stroke is this: Ask the person to 'stick' out their tongue. If the tongue is 'crooked', if it goes to one side or the other that is also an indication of a stroke. If he or she has trouble with ANY ONE of these tasks, call 911 immediately !! And describe the symptoms to the dispatcher.

Strokes have nothing to do with headache. With a stroke, you must have a neurological deficit for more than 24 hours. In her age this could be either a tension headache or a migraine.
